These 401k FedForms instructions cover only 401k related payments. Any 401k related income tax withholding for which you completed a Form 1099-R should be included in the year's Form 945.

Form 945 Filing Date(s): Monthly Deposits May Be Required

If the annual withholdings exceed $2,500, deposits to a financial institution must be made monthly and recorded on Form 8109 (with company name preprinted) or 8109B (for new plans, no preprinted company name). So long as such deposits are made on time and in full payment of the taxes for the year, Form 945 is not due to the IRS until February 11.

Form 945 is due to the IRS by January 31 along with a check for the amount reported accompanied by a payment voucher described below.

Use Form 945V to indicate to the IRS any payments you're sending in with your Form 945.

Unlike most of the other IRS Forms and Schedules, you can  print Form 945V from this website, if you wish (though the voucher is the second page of Form 945 as issued by the IRS, so if you've ordered and received Form 945 from the IRS you will have also received Form 945V).
