Annual Return of Fiduciary of Employee Benefit Trust
 Enter the appropriate plan year (as listed in your Adoption Agreement on page 2, item B3, with the appropriate year attached).
item 1a Name of trustee: see your Adoption Agreement page 3 item B6.
items 1b-c: List the address of your plan's trustee(s). As specified in your Adoption Agreement on page 3, item B6, this is likely the same as your Employer Address.
item 2a Name of trust: See your 401k plan administration software license information (Company Information, License Information).
item 2b: Enter your employer federal ID number, as recorded on page 1 of your Adoption Agreement, item A4. If you have established a separate plan identification number, use that number. The number you use should match that in your software’s License Information (Company Information, License Information).
item 3: Complete as needed.
item 4: Check “Yes” (unless you have not furnished such information).
item 5: Enter your company’s tax ID number. This is likely but not necessarily the same as the trust’s employer identification number that you entered for item 2b.
